Enrollment Information

Please complete online enrollment at: https://burbankusd.asp.aeries.net/AIR/. During the online enrollment, you will be prompted to upload the following documents. Please have a PDF or photo of the documents ready when you are completing the online enrollment.

  • Immunization Records (must be current)
  • Parent/Guardian picture ID
  • Proof of Residency (Burbank Water & Power bill AND Gas bill or landline telephone bill)

Immunizations Required for School Entry

  • DTP/DTaP/Dt/TD: 5 doses, but 4 doses meets requirement for ages 4-6 if at least one was on or after the 4th birthday.
  • Polio: 4 doses, but 3 doses are enough for ages 4-6 if at least one was given after the 4th birthday.
  • Hepatitis B: 3 doses 
  • MMR: 2 doses 
  • Varicella (Chicken Pox): 2 doses or licensed physician documented varicella disease or immunity.
  • NEW Additional Requirement: TB Risk Assessment: All newly enrolling students need to present a completed Tuberculosis (TB) Risk Assessment conducted and signed by a licensed healthcare provider. Please note: A TB test is not required for everyone. Your healthcare provider will determine if a TB test is necessary. Once you have completed the online enrollment application, you will receive an email with further instructions. All new enrollment applications will require an in person appointment for document verification. Please be prepared to bring the originals of the following documents:

  • State issued birth certificate or passport
  • Current immunization record
  • TB risk assessment completed by healthcare provider
  • Current Burbank Water and Power bill
  • Current Southern CA Gas bill
  • Parent id
  • If applicable, current report card or transcript

Enrollment is not complete until you have finalized registration during your in person document verification appointment. Enrollment is done on a first come, first served basis. If your school of residence is full, your child will be placed at another BUSD school. You will be placed on a wait list for your school of residence.
