Becoming a Cleared Volunteer
Any parent interested in helping in the classroom, going on a field trip, or volunteering at school events must be a cleared volunteer. Volunteer applications are submitted online.
As part of your volunteer application, you must upload the following documents.
- Your driver’s license. Please note that you will be required to bring your driver’s license with you the first time you volunteer at school.
- TB risk assessment form. Each volunteer is required to submit a Tuberculosis Risk Assessment and Certificate of Completion within the past 60 days. If risk factors are identified, an additional examination to determine that the volunteer is free of infectious TB will be conducted. This examination must be given by a physician, physician’s assistant or nurse practitioner. The TB test or Risk Assessment is valid for 4 consecutive years of volunteering. The school does not keep copies of TB documentation. The volunteer should keep a copy of their TB documentation for the 4 years it is valid. TB Risk Assessment Form
- Please read the Medical Provider Network Information, print and sign the final page of the document. You will upload this page to your online application. MPN Information and Signature Page
Once you have obtained the above requirements, applications are submitted online. Paper applications will NOT be accepted at the Providencia office or at the District Office. When choosing functions on your application, please do not choose overnight chaperone or transportation as they require fingerprint clearance. You will be notified by email through Raptor once you have been cleared to volunteer. Upon receiving clearance, you are welcome to contact your child’s teacher to set up a volunteer schedule.
